Do I need an order from my Primary Care Provider to see a PHYSical Medicine Provider?
All 50 states and the District of Columbia allow you to contact a physical therapist without a physician’s referral. (Your insurance policy may require a visit to the primary care physician first or limit your access to preferred providers only.)
If your physician refers you for physical medicine to be provided in the physician’s office, or to a facility in which the physician has a financial interest, know that you are not obligated to receive physical therapy in any specific facility.
You have the right to choose your own PHYSical Medicine Provider.
Do I need to be in pain before seeing a PHYSical Medicine Provider?
The short answer is No. You do not need to be in pain to see a doctor of physical medicine. PHYSical Medicine Providers are the movement system experts. We can help you prehab regardless of your levels of activity.
What will happen during my first physical medicine appointment?
If you were able to fill in your intake form well before your first appointment, your physical therapist, will have been able to thoroughly examined your medical history and should only need to ask some clarifying questions pertinent to the primary reason for you visit and establish the goals YOU want to achieve. Your PHYSical Medicine Provider will then conduct a comprehensive physical evaluation to gain a better understanding of your movement patterns and possible functional impairments. After your evaluation, your PHYSical Medicine Provider will discuss their findings and recommend a care plan to help you achieve YOUR goals.
The first PHYSical Medicine appointment will last approximately one hour.
What does deductible, co-payment and co-insurance mean?
A deductible is the dollar amount that must be paid by the insured prior to insurance cost-sharing. Patient is responsible for all allowable charges until deductible is met.
A co-payment is the dollar amount the insured is responsible for paying each visit. After the deductible is met, the patient is only responsible for the co-pay. PHYS|LAB collects co-payment at the time of service for all eligible plan participants unless other arrangements have been made in advance.
Co-insurance is a form of cost sharing. The co-insurance is a percentage of allowable charges the insured is responsible for paying each visit. After the deductible is met, the insurance will cost-share with the patient and will cover the portion of the allowable charges remaining after the patient’s co-insurance. PHYS|LAB collects a portion of co-insurance at the time of service for all eligible plan participants unless other arrangements have been made in advance.
Do I need insurance to be able to see my PHYSical Medicine Practitioners?
No. We accept all forms of payment from insurance, HSAs, credit cards, cash, etc. Just reach out to us and we can help you navigate your paymet structure.
Will I see the same PHYSical Medicine Provider on each visit?
Yes, one of the unique features of PHYS|LAB is that you will be seen by your physical therapist at every appointment. Your PHYSical Medicine Provider will service as your primary care provider for your physical wellbeing. We believe you will progress more quickly because your physical therapist will focus on your individual issues, be able to adjust your treatment plan within each session, and monitor your long term progress, as needed.
What should I bring to my first physical medicine appointment?
Make sure to complete your intake form prior to arriving and Plan to arrive about 10 minutes early. Bring your insurance information (i.e., insurance card, HSA card, etc) and your driver’s license or valid ID. Recent, related test results, such as MRI or X-ray reports, may be helpful and can be brought to your first appointment.
Wear comfortable workout clothing. It’s difficult to conduct a physical examination of your lower extremity when they’re covered by a thick fabric.
If at all possible, try to come for your first visit while symptomatic. If 1 mile of running tends to cause you pain, run 1 mile before showing up at the clinic.
Do you offer payment plans? Am I eligible for financial assistance?
In many cases, PHYS|LAB can offer an interest-free payment plan. Patients may set up a payment calling the Patient Advocate department at (949)390-9209.
